Help Me Help You Refer A Client, gives healthcare professionals the opportunity to refer eligible clients to access our essential and life-changing services.  Through a partnership with Unite Us, a national leading care coordination platform connecting healthcare and social service providers, HMHY also provides clients with a continuum of care by:
- referring clients to supporting services 
- providing client feedback to the referral partner
- ongoing measurement of the impact of HMHY services to improve health outcomes for individuals and the community.

Long Beach Food Pantry Delivery (LBFPD)
Free nutritious groceries are delivered weekly to the homes of eligible senior Long Beach, CA residents.


Benefits Access Support (BAS)
Enrollment assistance for social security benefits to include SSI/SSDI/CAPI, retirement benefits, disability benefits (physical and mental health conditions), child benefits, spousal/widow benefits, and appeals for SSI/SSDI/CAPI.
 


CalFresh
Free enrollment assistance for CalFresh benefit that provides qualifying low-income individuals with dollars via an EBT debit card to purchase groceries at participating grocery stores and farmers’ markets. 

Medi-Cal
Free enrollment assistance to access Medicare's comprehensive health insurance coverage at no cost or a reduced cost.
