Help Me Help You now offers FREE home-delivered, medically tailored groceries for Medi-Cal members who need nutrition that supports their health. Each package is customized for people managing chronic conditions - like diabetes, heart disease, high blood pressure, or kidney disease - or those healing after a hospital or skilled-nursing stay.

What Are Medically Tailored Groceries?

Groceries are selected based on your medical conditions and nutrition needs, following your doctor’s guidance. Packages may include:

• Fresh fruits & vegetables, lean proteins, and whole grains aligned with your care plan

• Condition-specific options (e.g., low-sodium for heart failure, carb-conscious for diabetes, kidney-friendly items when needed)

• Simple tips for preparing food at home

Who Qualifies?

You may qualify if:

• You have Medi-Cal coverage through LA Care, Health Net, Anthem Blue Cross Blue Shield, Molina, Inland Empire Health Plan, or CCCH, and

• You recently had a hospital or skilled-nursing stay, or

• You have one or more chronic conditions and face challenges getting, affording, or preparing healthy food

Our medically tailored grocery program is designed to:

• Make following your doctor’s nutrition recommendations easier

• Support better management of chronic conditions

• Reduce stress around food—especially for those who are homebound, have limited transportation, or live on a fixed income

What to Expect

If approved:

• A care manager or community health worker will confirm your eligibility and nutrition needs

• You’ll receive free regular grocery deliveries to your home within our service area (Long Beach and surrounding areas, including: Signal Hill (90755, 90807), Lomita (90717), Harbor City (90710), San Pedro (90731–90734), Carson (90745–90749, 90895), Wilmington (90744), Paramount (90723).

• .Our team may check in to support additional needs like benefits, housing, or transportation

 

How to Get Referred

This program requires a referral from your health care team or health plan.

To get started:

1. Ask your doctor, social worker, case manager, or health plan care coordinator if you qualify for “Medically Supportive Food” or “Medically Tailored Meals” under CalAIM Community Supports

2. Request a referral to Help Me Help You’s MediCally Grocery Delivery program in Long Beach, California.

 

Important Note

Program availability, eligibility, and length of services are determined by your Medi-Cal managed care plan and may vary.

If you do not qualify for the free MediCally Grocery Delivery, you are welcome to join our Long Beach Food Pantry Delivery Program for

a $10 delivery fee.
