Social Security Application Assistance
To apply for benefits, you should have a medical or mental health condition that warrants an application for disability benefits, or you are at least age 61 and 9 months and eligible to collect Social Security Retirement. If you are no longer able to work, and are considering applying for Social Security disability benefits, or have already applied and been denied, please contact us.
Individuals interested in applying for Social Security Benefits should first consult with their doctor for feedback or if there is a medical or mental health condition that warrants an application for disability benefits. It is helpful to have a detailed doctor's report if possible please bring this form with you to help determine if we can assist with the application process.
If we are able to assist, additional appointments will be scheduled with one of our team members for further assessment, and the client may enter our case management program to reach their goal of receiving benefits.
If you are working with another community agency that does not provide application services, the case manager should refer clients to us for assistance.
If we are unable to assist with the application process, individuals can always apply on their own for Social Security benefits at .
FREE representation is provided to clients who are homeless or at risk of homelessness. (Must meet the HUD definition of homeless). Services are also available to those who are low-income, elderly and frail, HIV positive, and adults and children with special needs due to mental health or physical health impairments.

Countywide Benefits Entitlement Service Team.
The Countywide Benefits Entitlements services Team is a Department of Health Services (DHS) program, comprised of eight (8) community based organizations with dedicated benefits advocates that can assist individuals with applying for the following disability programs: • Supplemental Security Income (SSI) • Social Security Disability Insurance (SSDI) • Cash Assistance Program for Immigrants (CAPI) • Veteran’s Benefits (VA). This program is offered to homeless and at-risk clients currently receiving MediCal benefits.

The SOAR Program provides assistance in helping clients apply for Social Security Income (SSI) and Social Security Disability Insurance (SSDI), programs administered by the Social Security Administration (SSA). Enrollment Agents can assist individuals with applying for the following disability programs: • Supplemental Security Income (SSI) • Social Security Disability Insurance (SSDI) • Social Security Retirement. This program is offered to non-MediCal clients.
*Please Note: We do not accept claims for over payments, nor do we accept new clients who already have representation, unless the former representative withdraws and waives his/her right to charge and collect a fee IN WRITING.
You must have a medical condition that meets Social Security Administration’s definition of disability in order to obtain disability benefits. In general, SSA disability benefits include a monthly cash payout and medical care in the form of Medicaid or Medicare.
